Description
Please use your real name in the First and Last Name fields. The Middle Name is optional.
If you prefer to go by a different name, add that into the badge name fields. You may use one or both. Whatever is in those fields will replace your First, Middle, and Last names on the badge. (Example: If your first name is Robert or Bobbie and you go by Bob, you can go ahead and put Bob in the First Name field. If you put Bob in the Badge Name field, only the name Bob will show on the badge. In that case if you want your last name to appear on your badge, please add it to the Badge Name field(s).)
